I don't think there is a mandate for recurring W-4 notices, however, it could be different by state. Nonetheless, I think sending an annual W-4 notice or reminder is a good practice. I've deposited a W-4 Annual Update Letter in the HR Resources page that you might want to reference or use as a template.

We recently had an employee get assessed for insufficient withholding. He of course pushed the blame to our HR department suggesting we failed to inform him of proper W-4 updating. Does anyone know if W-4 update notices are either common or required?
